Surveys are a great way to gauge how members feel about current services and what they would like to see in the future.
Surveys can be short polls or long questionnaires.
How are Surveys created?
Surveys are created as interactive content.

Interactive Post: if the survey period has a start and end date

Interactive Page: if the survey does not have an end date and you want members to come back and resubmit periodically.
Design

Interactive Features
The interactive features that are useful when creating a survey are:

Include Company: Helpful for identifying where the member is coming from.

Time Reset: Used when creating a permanent surveys (as Pages), this resets when the member can submit another survey response.
Data Inputs
The main purpose of a survey is to obtain usable information in a format that can be analyzed easily.

Text input: for names, single line answers, multi-line answers.

Number inputs: 3 types of ways to input a number.

Contact information: fields designed to accept email addresses, phone numbers and more.

Date and time: several ways for a member to enter a date, time, date range, time range.

Selectboxes, checkboxes, radio boxes: several ways for a member to choose from various options.
Interactive Block
Once you have configured the Interactive Features and Data Fields as needed, it will then appear in the Layout tab of the Editor where it can be dragged & dropped into the canvas as a custom block.
Workflows & Notifications
Workflows can be used to automate messages to the member after submission. For a survey, you can create workflows for:

Confirmation of submission.

Notifications to submit before the survey period ends.
Results
Once the survey period has ended, you can download the results from the Table tab of the interactive content item:
You can analyze the results using a spreadsheet or database app.
